Thank you for your interest in becoming a Patient Advocate for HJAHC Health Connections. The mission of this program is to improve the social determinants of health for our patients and positively impact their overall well-being. By mitigating systemic barriers to health and promoting health equity, we aim to create a healthier and more equitable community. Health Connections is based on the successful evidence-based Boston Health Leads program, which was founded in 1996 by the Boston Medical Center Pediatrics Department to address the impact of poverty on health. We are proud to launch this initiative and are committed to making a difference in the lives of our patients. This program is a partnership with the Pace Center for Civic Engagement and is only available to Princeton University students at this time.
Mandatory requirements (please read carefully):
• Weekly 3-hour Help Desk shift
• 1-hour monthly group reflection session (with Princeton University Project Leaders)
• All screenings, encounters and follow-ups with patients must be logged and updated.
• If accepted, new volunteer training will be offered on September 30th from 10 a.m.-2 p.m. This is the only date this semester and volunteers must be able to attend this session to participate in this program.
• HIPAA training and medical clearance in a timely manner is a must. Applicants should have ready access to their own immunization records. The process begins immediately after acceptance into program.
• Once accepted into the program, your application is valid for one semester, with the option to renew into future semesters.
